If you are already a member you will not have to fill EVERYTHING out, however we want to make sure we have your most up-to-date information and you are required to fill out the top portion of the form. Thank You!
Every cast and crew member of a production must be a Theatre 7 member with annual dues paid for the current season. Members under 16 years old must have a guardian sign as a representative.
What areas of theatre are you potentially interested in?
I UNDERSTAND that I am required to become a member of Theatre 7 and pay $25 annual dues for the current season in order to perform or assist backstage in any shows and be covered by insurance (ushers and paid positions are not required to be members). For families with multiple children in a production, annual dues will be discounted. I consent to receiving emails and occasional physical mail from Theatre 7 and understand that I can opt out at any time. I release Theatre 7 to use my image in any promotional photography or video and in perpetuity. For guardians: I AGREE to serve as a representative for this member during the current season.
